The Community Inclusion Project announces the availability of a training manual for creating care-coordination plans for individuals with dual diagnosis
A treatment model integrating health, psychiatric, and behavioral services for people with dual diagnoses transitioning from Middle Tennessee Mental Health Institute (MTMHI) to the community is now available. The Community Inclusion Project (CIP) produced the manual and coordinated the demonstration project through the Vanderbilt Kennedy Center Behavior Analysis Clinic. Funding sources include the DMRS, MHDD, and TN Council on DD. The goal of the manual is to identify which professionals are responsible for services, and how to work within the framework of the policies governing healthcare for people with dual diagnoses. A second goal of the manual is to provide care providers with an understanding of how independent support coordinators, behavior analysts, psychiatrists, and medical professionals can work together to reduce barriers that influence the quality of life for consumers.
